Our aim is to empower them with the means to effectively manage their condition.
The primary objective of our IBD Patient Navigator™️ Program is to enhance the quality of life for marginalized communities, including African Americans and other underserved populations, who are struggling with Inflammatory Bowel Disease (IBD). By forming a cohesive link between:
- IBD Patients
- Healthcare Specialists
- Primary Care Physicians
- Mental Health Professionals
- Health Insurance Companies
This program advocates to assist IBD Patients with getting equitable access to quality healthcare and educational resources.
The IBD Patient Navigator™️ Program extends its outreach through a series of community lunch and learn sessions, workshops, and seminars designed to empower and educate underserved IBD populations afflicted by Crohn’s Disease or Ulcerative Colitis. These initiatives are geared towards fostering greater self-management of their health and well-being.
What is an IBD Patient Navigator™️?
An IBD Patient Navigator™️ is a patient advocate who helps individuals diagnosed with Inflammatory Bowel Disease (IBD) navigate the healthcare system and manage their condition.
Our team of IBD Patient Navigators™️ serves as intermediaries, effectively bridging the gap between patients, healthcare professionals, and community agencies. We achieve this by offering comprehensive educational resources pertaining to Inflammatory Bowel Disease (IBD).
This includes:
- Assist IBD patients with communicating with their healthcare providers so they get the information they need to make decisions about their health care.
- Assisting IBD Patients with getting equitable access to quality healthcare.
- Advocating for patients to ensure they have access to the appropriate treatment options.
- Address healthcare treatment concerns, and any financial and healthcare disparities.
- Partnering with Health Insurance Companies, Case Managers, Healthcare Professionals who may have an effect on an IBD patient’s healthcare needs.
The IBD Patient Navigator™️ Program is steadfast in its dedication to providing underserved individuals living with Inflammatory Bowel Disease (IBD), particularly those diagnosed with Ulcerative Colitis and Crohn’s Disease, with the essential knowledge necessary to facilitate effective communication between themselves and healthcare professionals.
